The premise hinges on the volatile border between reality and a fictional webtoon. Han Hyo-joo plays a surgical resident who enters a hand-drawn universe to confront its protagonist. This interaction creates a loop where the narrative choices of her father dictate the brutal reality of the comic world, forcing the characters to navigate frequent shifts between a modern medical practice and a landscape defined by attempted murder and gun violence.

The tone balances dark themes and light-hearted comedy through the impossible romance between a resident and a wealthy chaebol who exists only on the page. Because the hero fights to maintain agency over his own scripted destiny, the series treats its fantasy setting with consistent stakes. Every transition between worlds brings new threats to the couple, keeping the romance secondary to the fundamental struggle of characters attempting to break free from their creator.
